Swim Class Makeup Request

Please make sure you notify us in advance if you plan to miss a class!

Please use this form to request a day/time/class. Please DO NOT SHOW UP AT CLASS
until you have a confirmation from the instructor as many of our classes are full so there
is no room to take on additional students for a makeup. Please allow 7-10 days to get
this scheduled.
